Health Catalyst’s mobile offerings aim to improve Care Management by providing tools for patient stratification, administration, coordination, and engagement. The Care Management suite of products gives health systems and healthcare providers the tools they need to run their Care Management and Population Health initiatives effectively.

Health Catalyst’s mobile apps include Care Coordination (iPad) and Care Companion (iOS and Android). Care Managers are given the tools requisite to manage their patients’ needs. This includes coordination of care teams, scheduling, medication planning, and secure messaging with patients. Patients are given the ability to view their care plan, receive reminders, securely message with their care team, view upcoming appointments, and more.

Update: The Care Management product line was ultimately sunset after Health Catalyst’s IPO in 2019.
